Glasses and headwear are the most common reasons for rejection.
Remove glasses before taking your photo. Glare, reflections, or frames covering the eyes are the single most common reason Japanese visa photos get rejected. If you must wear glasses for a documented medical reason, you'll need to submit supporting evidence.
Head coverings are generally not allowed. The only exception is for religious purposes, and even then your full facial outline from chin to forehead must remain visible with no shadow cast on your face.
